Remote Patient Monitoring System "Mano Ritmas"
"Mano Ritmas" is a Remote Patient Monitoring (RPM) system designed to support patients participating in health wellness, disease prevention, and rehabilitation programs. The system helps monitor adherence to treatment or wellness plans, assess changes in health indicators, and maintain continuous communication between patients and healthcare professionals.
The patient mobile application reminds users of scheduled tasks, collects data from smart devices and medical equipment, and transmits this information to healthcare professionals in real time. Healthcare providers can centrally monitor patient status, analyze collected data, and respond promptly to significant changes in health conditions.
The system helps improve patient engagement, increase adherence to treatment plans, and optimize the organization of healthcare professionals' work.
The mobile application provides patients with a single platform for daily health-related tasks, health monitoring, and communication with healthcare professionals.
The application helps patients follow their treatment or wellness plans by reminding them to perform scheduled measurements, allowing them to record daily fluid intake, enabling nutrition and medication diary tracking, reminding them to complete questionnaires.
Patients can view the history and trends of their health indicators through charts and graphs. Depending on the assigned care plan, the following metrics can be monitored: heart rate, blood pressure, oxygen saturation (SpO₂), body weight, blood glucose levels, step count, active minutes, sleep duration.
The healthcare professional portal enables centralized patient management, health monitoring, and treatment coordination.
The system supports automated collection of health data from medical devices and wearable technologies, including:
- Microlife BP B6 blood pressure monitor;
- Garmin smart devices;
- Teltonika TeltoHeart medical wristband;
- other devices through Android Health Connect and Apple Health integrations.
Automatic data transfer reduces the need for manual data entry and ensures that healthcare professionals always have access to the most up-to-date information about a patient's condition.
System capabilities
The patient record allows users to view and manage contact information, diagnoses, allergies, medication history and prescriptions, the health plan and its implementation, as well as the history of health indicators. It is also possible to upload test results, schedule consultations, complete consultation notes, and store documents related to the patient.
The system complies with the requirements for Class I medical devices established by Regulation (EU) 2017/745 on medical devices.
When creating a health plan, it is possible to select which plan components will be applied to a specific patient: nutrition diary and recommendations, water intake tracking, activity goals, exercises, health indicator measurements (tracked indicators, measurement frequency, and critical indicator thresholds), and periodic questionnaires.
Patients receive reminders in the application about tasks assigned according to their health plan, including medication intake, required health indicator measurements, upcoming consultations, and other planned activities.
The specialist system allows users to review current-day tasks and track the completion progress of tasks from previous days. If a patient fails to complete assigned tasks, the system automatically generates a notification for the case manager, allowing them to follow up and take appropriate action in a timely manner.
Integrations with wearable devices and medical equipment enable automatic collection of heart rate, blood pressure, oxygen saturation, step count, and ECG measurement data. This reduces the need for manual data entry and improves data accuracy and reliability.
Patient-entered data (manually entered or collected from connected devices) is displayed in the system using charts and tables, enabling convenient monitoring of health indicator trends and historical data.
Specialists assigned to a patient can communicate with them in real time using a chat function. When needed, each specialist can enable or disable the patient’s ability to initiate conversations, helping to manage communication flow and ensure effective interaction.
The system allows specialists to create tasks for themselves or for other team members. When creating a task, users can select its type, assign the related patient, and set a completion deadline.
